I have both PayPal and manual checkout selected. When the customer adds and item to their cart and if they select manual checkout they are being charged shipping. Is there a setting that I missed that disables the shipping charge when an item is added to the cart and the customer selects manual checkout?

Please post a link to your products page so I can go there and add an item to the cart and checkout the behavior.
Have you added shipping for the items that you are adding to the cart?
The product link is http://lynnmuseum.com/shop1/fluffshirts/. Yes I have added a base shipping cost in the settings page. I also have have an amount in the Item shipping cost.

I see... so you basically want to make it so for *Your* manual checkout it won't charge shipping right? I thought the plugin was adding shipping incorrectly when you go to the manual checkout.
I have added an option in the "Manual Checkout Settings" which should allow you to do what you are after. Please get a new build of eStore from here and check the "Do Not Add Shipping Charge" field from the manual checkout settings:
