Is it possible to set-up lifetime commissions to an affiliate e.g. all sales generated by the customer the affiliate refers pay a commission for life.

    OR
Even better, is it possible to define the length of time an affiliate would be paid a commission. E.g. commissions will be paid for the next 12 months from anyone the affiliate refers.

Yes, you control this using the "Cookie Lifetime" value in the settings. Please read the documentation and the help text for this field which will explain more.

Sorry, I should have clarified a little further as I had read about and seen the cookie lifetime field.

If the customer who was referred clears their cookies from their browser though the tracking will be lost? Is this the only form of tracking? i.e. there is no other way for the plugin to store the referrer (affiliate) against the customers details to ensure repeat sales are tracked, even if they clear their cookies.

I see what you mean. No an affiliate does not own a customer for life. The customers are anonymous (until you make them register for a member account and track them via their membership). The only way you can track someone anonymous (without invading their privacy) is via a cookie. Some more examples of this on this tread:
