The scripts and HTML on your webpage is broken badly. I just validated the following page using w3 validator (http://validator.w3.org/) and there are 87 errors on the page! Not all errors are extreme but some are.

So whats happening is that you have many unclosed HTML tags that haven't been closed in that page which is completely throwing the ajax script tag of the squeeze page.
You need to start cleaning up the coding violations a bit then you will see the squeeze page form will appear nicely.
