I do have the "Send Notification to Admin" checked. An email has been put in for "Contact Email Address".
I tested by creating an affiliate. As an affiliate I did receive the confirmation email, however the notification email to the admin did not come through.
We're using WPAP 4.0.4 and WordPress 3.0.4.

Make sure your from email address and the admin email address is not the same. If the "from" and "to" email addresses are both the same then some server can reject to send that email.
In the "Email Settings" tab there is a "From email address" field. Is the email address used in that field same as the admin email?
